Ear Wax Removal: Microsuction vs Irrigation — Which Is Right for You?

Ear wax is completely natural and usually works its way out on its own. But sometimes it builds up and causes blocked ears, muffled hearing, earache, or tinnitus. When that happens, professional removal is safe, quick, and effective. But which method should you choose?

Why Do Ears Get Blocked?

Your ears produce cerumen (ear wax) to protect the ear canal from dust, bacteria, and water. Some people naturally produce more wax than others. Common causes of build-up include:

  • Regular use of earphones, hearing aids, or ear plugs
  • Narrow or hairy ear canals
  • Using cotton buds (which push wax deeper)
  • Working in dusty environments
  • Age — wax becomes drier and harder as we get older

Microsuction: The Gold Standard

Microsuction uses a small suction device and a microscope or loupe to gently remove wax from the ear canal. It is widely regarded as the safest and most effective method.

Advantages:

  • No water involved — safe for perforated eardrums and ear infections
  • The clinician can see exactly what they are doing throughout
  • Quick procedure — typically 15-30 minutes for both ears
  • Minimal discomfort
  • Can work on hard, impacted wax

What to expect: You will sit comfortably while the clinician uses a gentle suction tip to remove the wax. You may hear a noise from the suction, but the procedure is painless for most people.

Irrigation: The Traditional Approach

Ear irrigation (formerly called syringing) uses a controlled flow of warm water to flush wax from the ear canal. Modern electronic irrigators are much safer than the old metal syringes.

Advantages:

  • Effective for soft or partially loosened wax
  • Comfortable — the warm water is soothing for many patients
  • Well-established technique with a long history

Not suitable if you have: A perforated eardrum, ear infection, previous ear surgery, or grommets.

How to Prepare

For best results with either method, use olive oil ear drops for 3-5 days before your appointment. Place 2-3 drops in the affected ear twice daily while lying on your side. This softens the wax and makes removal easier and more comfortable.
