About the Service

What Is the Discharge Medicines Service?

When you leave hospital, your medicines may have changed — new ones added, doses adjusted, or medicines stopped. This can be confusing. The Discharge Medicines Service (DMS) is a free NHS service where our pharmacists review your medicines after hospital discharge to make sure everything is correct and you understand your new regime.

What Happens After Discharge

Our pharmacist reviews your discharge summary, medicines list, and any changes made in hospital.
